Please provide the following information and materials

1. Information for the fly, include you, your source for materials, especially if they are unusual or not readily available:

History of the fly, how developed, why, etc. (type on another page if you wish)

2. The name of the fly shop where you get Fly Fish America OR the name of your shop if you distribute the magazine. Please include telephone numbers and contact person if appropriate.

3. Send at least one fly in size #10 or larger (for photographic reasons). We do understand some flies are only fished in #20 and smaller but they are difficult to photograph in the small sizes; #10 or larger please.

4. Any personal information we may consider for publication.
