especially if they are unusual or not readily available: Fly Name ________________________________________________________________________ Materials List: (type on another page if you wish) Your materials source: (type on another page if you wish) History of the fly, how developed, why, etc. (type on another page if you wish) 2. The name of the fly shop where you get Fly Fish America OR the name of your shop if you distribute the magazine. Please include telephone numbers and contact person if appropriate. 3. Send at least one fly in size #10 or larger (for photographic reasons). We do understand some flies are only fished in #20 and smaller but they are difficult to photograph in the small sizes; #10 or larger please. 4.
